FAQ: approvals, evidence and who sees what

Common questions about what the agent does on its own, what needs a person, and who is alerted.

Short answers to the questions safety teams ask most in their first weeks.
1

Does the agent do anything without approval?

It writes reports, drafts root causes, proposes actions, sends reminders and assembles evidence. Assigning actions, closing them and sharing anything outside the team need a person.

2

Who is alerted when something is reported?

The people you set for that site and incident type. Routing is configured with you during rollout and can be changed at any time.

3

Why does an action need a photo to close?

Because actions marked done without proof are the most common audit finding. The photo is the evidence, and a person still has to accept it.

4

Can a supervisor report without an account?

That depends on how your workspace is set up. Many teams give every supervisor a phone login; some use a shared reporting link for near-misses.

5

What if the agent gets a report wrong?

Correct it. The report is editable before and after it is routed, and the agent learns from the corrections.
