Review a drafted root cause

What the agent drafts from the report and photos, and what to check before you accept it.

3 min read
After an incident is reported, the agent opens the investigation and drafts a root cause from the report, the photos and similar past incidents. It is a starting point for the investigator, not a conclusion.
1

Open the investigation

The investigation is linked to the incident report. You see the facts the agent used and the root cause it proposes.

2

Check the facts

Confirm the sequence of events, the location and the people involved. Add anything the reporter left out, such as a witness statement.

3

Challenge the root cause

Ask whether the proposed cause explains why the condition existed, not just what happened. Edit it if it stops at the immediate cause.

4

Accept it

When you accept the root cause, the proposed corrective actions are tied to it. The investigation record stays with the incident.

Bring a recent incident report and we will walk through how it would be captured, investigated and closed.
