What counts as "Live task observation" versus "Record review only"?
Live task observation means the inspector is physically present while an authorised employee performs an actual isolation. Record review only means the inspector checked paperwork or a prior log without watching anyone work. The template scores these differently on purpose: a record review tells you nothing about whether the isolation is done correctly, only that a form exists.
What does "Verification Performed" mean here, distinct from "Try Test Performed"?
Verification Performed asks whether the worker confirmed isolation before starting, by any method. Try Test Performed asks specifically whether they attempted a normal start after applying locks, the one method that actually proves the energy is gone. Yes on the first without a yes on the second usually means someone checked a gauge, not the machine's actual response.
Why does the template ask if the inspector is authorised, separately from who applied the lock?
OSHA requires the periodic inspection to be conducted by an authorised employee other than the one utilising the procedure being inspected. If the inspector isn't authorised for that specific procedure, the observation doesn't satisfy the regulation regardless of what they saw.
Scope
When is a loto periodic inspection required?
This inspection is one step in a larger programme. Using it for work belonging to a neighbouring template produces records that are hard to report on later.
Use this template when
- An authorised employee is about to, or has just, performed an actual isolation on the machine covered by the procedure
- The inspection frequency set in your programme has been reached, at least yearly per procedure
- The asset or area covered by the procedure has changed since the last inspection
- You are running the Lockout and Tagout programme and this is its yearly verification step
- A linked record needs this one to exist, such as an asset record or the procedure it verifies
Do not use it for
- Lockout/Tagout Procedure (Machine Specific), which sets out exactly how to isolate a machine, listing every energy source and isolation point.
- Energy Isolation Verification, which records the physical test proving stored energy is gone, on a single occasion rather than a periodic cycle.
- Group Lockout Record, which records a group isolation where several people work on one machine under a single lockout device.
- Lock Removal Authorization, which records who may remove a lock they didn't apply, a narrower decision than this inspection makes.

LOTO Periodic Inspection requirements by country
Lockout and tagout is a US OSHA construct by name, but the underlying principle, deliberate verified isolation before exposure to hazardous energy, is enforced wherever machinery is regulated. What differs is which document sets the annual verification requirement.
OSHA 1910.147(c)(6)
A mandatory, named periodic inspection, at least annually per procedure, by an authorised employee other than the one who uses it.
The observation and authorisation check aren't best practice, they're the specific thing the regulation names, so an unauthorised inspector is a compliance gap, not a quality issue.
CSA Z460
The national control-of-hazardous-energy standard, referenced by provincial regulators, built around the same authorised-verification concept as the US rule.
Provinces adopting CSA Z460 expect the same evidence this template produces: a dated observation by someone qualified to judge the procedure, not a signed checklist.
ISO 14118 / EN 1037
Sets the isolation and prevention-of-unexpected-start-up principle technically, but leaves the frequency and ownership of periodic verification to national law.
Sites outside US and Canadian jurisdiction still need periodic verification for defensible evidence; this structure works even where no regulator names it directly.
How to complete it
How to complete a loto periodic inspection, step by step
Filling in every field isn't the same as producing a defensible record. These are the calls that decide whether the inspection proves what it claims to.
An inspector who arrives after the work and fills the form in from memory or from asking the worker isn't conducting a periodic inspection, whatever Observation Type says. If the timeline doesn't support a live observation, the record should say so rather than default to the option that scores best.
Someone can be a competent, authorised LOTO practitioner in general and still not be the right person to inspect a procedure they weren't trained against. Inspector Authorised should be answered against the specific procedure, not the person's general standing.
Correct Shutdown Sequence and Correct Restoration Sequence both allow a middle answer, meant for genuinely trivial ordering differences, not for skipped steps that happened not to cause a problem this time. If in doubt, score it as a failure and let the finding carry the detail.
Procedure Accuracy can fail because the worker didn't follow the documented steps, or because those steps no longer match the machine. Retraining fixes the first; only a revision against MNT-055 fixes the second, and closing the inspection without that distinction leaves the wrong corrective action in place.

Case in point
Case in point: the inspection that passed on paper
A site ran its LOTO periodic inspections on schedule for two years, every record marked Complete, Result Band Pass. When an auditor asked to see one in progress, the inspector admitted they usually reviewed last year's paperwork and the worker's training record, because scheduling a live observation around production was awkward.
Every field had an answer. None of them proved a live isolation had ever been watched. The fix was making Observation Type a field the safety lead reviewed before accepting any record as closing the annual requirement, and treating Record review only as automatically incomplete rather than a valid pass.
