What is a contractor work completion record?
It is the document that closes out a job at handover: it confirms the work matches the specification, the area's safety systems and housekeeping have been restored, and any permit the job ran under can be closed. It exists so that 'the contractor says they're done' becomes something verified rather than assumed.
Who should sign it?
Two people: someone from the contractor who did the work, and the area owner accepting it back. A single signature from either side turns the record into a report rather than a handover, because the point of the second signature is that the person taking the area back has actually checked it.
How is it different from closing the permit itself?
The permit closes against its own conditions — isolations, time limits, the specific hazards it was written for. This record is broader: it covers whether the work met specification, whether the area is safe and clean, and whether performance should feed the contractor's scorecard.

When is a contractor work completion record required?
A work completion record is the last gate before an area returns to normal operation, and it is easy to compress it into a formality once the job looks finished.

- A contractor's task, work order or job on this site has reached the point of handover

The duty behind this record is the organisation's obligation to control contracted work, not a single closure regulation, so jurisdictional differences show up in which underlying hazard standards apply rather than in the handover format itself.
OSHA 29 CFR 1910.147 and 1910.212
No federal standard governs a completion record itself; hazardous energy control and machine guarding are the enforceable duties the restoration fields evidence.
Answering 'guards refitted: yes' without a functional check leaves the underlying OSHA duty unmet even though the form is complete.
Management of Health and Safety at Work Regulations 1999, reg.3, and PUWER 1998
Employers must ensure work equipment is safe to use after work on it, and that risks introduced by contractors are controlled.
The area owner's signature is where that duty is actually discharged, not the contractor's own assurance that the job is done.

Contracted work must be controlled to the same standard as the organisation's own work, with verification rather than reliance on the contractor's assurance.
Certification auditors look for evidence the organisation checked the work, not just that the contractor stated it was complete.

How to complete a contractor work completion record, step by step
The template prompts for a long list of restored items. What determines whether the sign-off means anything is whether those items were checked or assumed.
Guards, interlocks, emergency stops, locks and isolations determine whether the area is safe, not the general 'work completed as specified' line. Signing off from memory, rather than a walk-through and a function test, is where incidents afterwards actually originate.
Debris, swarf, screens and covers are specific to engineering work in a food handling area, and a general 'site left' check tends to skip them because they look like housekeeping rather than safety. Where a pre-operational inspection is required, handback should wait for it rather than assume the area looks clean enough.
'Anything unaccounted for' only functions as a control if reconciliation is a count against a pre-work inventory, not a tick made from memory at the end of a long job. A missing item left inside equipment is the predictable outcome of skipping this, not a rare one.
A completed job with outstanding items, late documentation or a disputed snag list is not 'excellent' performance, whatever the tone of the handover conversation. This record feeds the scorecard, and inflating it here removes the earliest signal that a contractor's standard is slipping.

Case in point: the completion record that closed a job nobody had actually inspected
A mechanical contractor finished a pump seal replacement in a wash-down area of a food plant. The record shows guards refitted yes, isolations restored yes, area cleaned yes, tools and parts reconciled yes, and both signatures present, closed within twenty minutes of the contractor finishing.
Three weeks later a screwdriver turned up inside the guard during a routine service, traced to that job. The area owner had signed by phone, taking the contractor's word for the walk-through, and reconciliation had been ticked from memory rather than counted against the kit list. The fix was not a longer checklist; it was making the second signature conditional on a logged physical walk-through, not assumed inside the signature itself.

Frequently asked questions about contractor work completion record
Who should sign the work completion record?+
The area owner accepting the work back, and a second signature from the contractor's supervisor. Both are required because the record's purpose is independent verification, not a single party's account of the job.
Does the area owner or the contractor sign first?+
Order matters less than sequence: the area owner's signature should follow a physical check of the area, not precede it based on the contractor's assurance that everything is done.
What happens if there is an outstanding item at handback?+
It goes in the outstanding items field with the snag list marked accordingly, and the area can still be handed back conditionally if safe to do so. What should not happen is the item disappearing once the record is closed.
Should performance be assessed on this record or wait for the scorecard?+
Here, at the point the job closes, while the detail is fresh. The scorecard aggregates this and other inputs over a period; if this record is not honest, the scorecard inherits the same blind spot.
What if a tool goes missing during reconciliation?+
Mark it unaccounted for and raise the linked case rather than closing the record and hoping it turns up. In a food production area, an unaccounted-for item is a foreign-body risk until found or ruled out.
Does the permit close automatically when this record is signed?+
No. This record confirms conditions that allow the permit to close, but the permit itself closes against its own specific conditions in its own record.
