What is a transport provider assessment?
It is a structured review of a haulier's licensing, driver management, vehicle standards, cold chain capability and load security, used before a contract starts and repeated yearly. It sits under ISO 45001 cl.8.1, which requires control over processes carried out on the organisation's behalf, and it feeds the Supplier Quality Assurance and Fleet and Driver Compliance programmes.
Why does a haulier need a separate assessment from a general contractor check?
A general contractor assessment covers site conduct and competency; it does not test whether a vehicle can hold temperature, secure a mixed load or survive a roadside enforcement stop. Those are transport-specific failure modes that only surface when someone checks the operator licence, the reefer maintenance record and the securement standard together.
What makes this a singleton record rather than one per delivery?
The assessment establishes whether the haulier is fit to carry your product at all — it is a standing judgement on the relationship, not a record of any one journey. Individual trips are covered separately by load tender, delivery point and temperature records that reference this assessment rather than repeat it.

When is a transport provider assessment required?
This assessment is one step in a larger programme. Using it for work that belongs to a neighbouring template produces records that are hard to report on later.
Use this template when
- A new haulier is being approved to carry your product, before the first load moves
- The annual review has come round for a haulier already on the approved list
- The haulier's operator licence, insurance or subcontracting position has materially changed
- You are running the Supplier Quality Assurance or Fleet and Driver Compliance programme and this is one of its steps

Do not use it for
- Refrigeration Contractor Verification, which verifies the contractor who services your own refrigeration equipment, not a haulier's vehicle fleet.
- Cleaning Contractor Verification, which verifies a sanitation contractor's chemical control and isolation competence, a different service entirely.
- Waste Contractor Verification, which verifies waste carrier registration and destination permits, not transport of your own product.
- Reefer Unit Check, which checks one refrigeration unit's operation on the day, not the haulier's overall cold chain capability.

- Subcontracting Declared, which is left as No subcontracting because nobody asked the haulier directly.
- Score Percent, which reads high because failed items were marked N/A instead of scored as failures.

Transport Provider Assessment requirements by country
ISO 45001 is the management-system spine, but a haulier's legal right to operate a vehicle at all is set locally, and it is worth checking against the right instrument for where the vehicle is registered.
Operator's licence under the Goods Vehicles (Licensing of Operators) Act 1995
A haulier cannot lawfully run goods vehicles over 3.5 tonnes without a standard operator's licence issued by the Traffic Commissioner.
Operator Licence Current is not a formality field — a lapsed or revoked licence makes every subsequent journey with that haulier an offence you are commissioning.
Regulation (EC) 1071/2009 on access to the occupation of road transport operator
Sets common conditions — good repute, financial standing, professional competence — that member states must enforce before granting an operator authorisation.
A haulier operating across EU borders should hold a Community Licence under this Regulation; its absence is a licensing gap the same as a missing domestic O-licence.
FMCSA operating authority and safety rating under 49 CFR Part 385
Motor carriers need FMCSA operating authority, and their safety rating (Satisfactory, Conditional, Unsatisfactory) is public and tied to inspection and crash history.
Enforcement History Acceptable should be checked against the carrier's actual FMCSA safety rating, not the haulier's own summary of it.
How to complete it
How to complete a transport provider assessment, step by step
Filling in the fields is mechanical. The judgement calls are what decide whether the finished assessment would survive someone else reading it cold.
A Yes on Refrigeration Units Maintained or Insurance Adequate For Load Value should trace to a document actually seen — a service schedule, a policy schedule — not a verbal confirmation. Decide this threshold once, in writing, so assessors do not apply different bars.
Declared subcontracting sits deliberately between No subcontracting and Not disclosed. The judgement call is whether a declared subcontractor needs its own mini assessment, or rides on the prime haulier's approval — settle that before a load goes missing, not after.
This template judges the haulier's systems and history, not one vehicle. A haulier can score well here and still send a trailer with a defective coupling — that failure belongs to Trailer Inspection or Reefer Unit Check, not this record.
A Caution on a knockout-style item — no operator licence, no temperature recording fitted — should not average out against strong scores elsewhere. Decide which fields are load-bearing before the first Caution forces the question under pressure.

Case in point: the approved haulier with the unapproved trailer
A haulier passes its annual transport provider assessment comfortably — licence current, insurance adequate, cold chain fields all Yes based on the transport manager's summary. Three weeks later a delivery arrives two degrees warm because the trailer actually used that day was a spare unit, borrowed from another depot, with a recorder that had not been calibrated in over a year.
Nothing on the assessment was false; it described the haulier's fleet in general, not the one trailer on the road. The gap was treating a company-level Yes as coverage for every vehicle the company might send, when the assessment was never designed to reach that far down.

- Cold chain
- The unbroken sequence of temperature control from production through storage, transport and delivery. A single weak link — including a third party haulier — breaks the whole chain.
- Operator's licence
- The legal authorisation, granted nationally, that permits a company to run goods vehicles above a set weight. Its absence or revocation stops the haulier operating lawfully at all.
- Excursion
- A period where product temperature moves outside its defined safe range. What matters is not just that it happened, but whether the haulier's process caught and reported it.
- Subcontracting
- Where the haulier you approved passes the job to another carrier. Undisclosed subcontracting means the vehicle and driver on the road were never assessed at all.
- Load securement
- The equipment and technique used to stop cargo shifting in transit. Competence here is about training and evidence, not just the straps being present on the vehicle.

Frequently asked questions about transport provider assessment
Does a good score mean every load will arrive intact?+
No. It means the haulier's systems and general capability were sound at assessment. Individual journeys still depend on the specific vehicle and driver on the day, covered by separate trip-level records.
Who should complete this assessment?+
Logistics leads it, with quality signing off given the cold chain and food safety stakes. Neither function alone tends to catch both the licensing gaps and the temperature gaps.
What happens if a haulier fails the annual reassessment?+
Action Required should be set to Yes, a CAPA raised, and the haulier's status reviewed before the next load is booked — a Fail here is a supply decision, not just a form to file.
Should every haulier get the same assessment regardless of load type?+
The fields apply generally, but the weight given to cold chain items should differ for ambient-only versus temperature-sensitive freight, or assessor time gets spent on irrelevant questions.
