The register is where confined space management succeeds or fails, because every control downstream depends on a space being listed and correctly classified. A space nobody has identified is entered without a permit, without atmospheric testing and without rescue arrangements, and the person who discovers that it is dangerous is the person already inside it.

What is a confined space register?

A record of every confined space on site, with its classification, the hazards present, the isolation points, access restrictions and rescue considerations. It is the source document from which entry permits, procedures, signage and training are derived.

What makes a space permit-required?

It meets the definition of a confined space and contains or has the potential to contain a hazardous atmosphere, contains material with the potential to engulf, has an internal configuration that could trap or asphyxiate, or contains any other recognised serious safety or health hazard.

Two activities produce a reliable register: walking the site, and getting the classification right.

Identify by physical survey, not from drawings

Drawings show designed vessels. They do not show the pit created when a floor was lowered, the void behind a tank installed against a wall, the duct that became accessible when a panel was removed, or the chamber that was always there and never labelled. A physical walk with someone who knows the definition finds spaces that no drawing set contains.

Classify against the definition, deliberately

Confined space or not; if so, permit-required or not; if permit-required, is alternate procedure entry available or has every hazard been eliminated. Each step has criteria and each produces different obligations. Recording the reasoning matters as much as the conclusion, because the classification will be questioned after an incident.

Record what an entrant needs before entry

Hazards present, isolation points, previous contents, access dimensions and orientation, whether retrieval equipment can physically be used, and rescue considerations. That information determines whether an entry can be planned safely and is otherwise reconstructed at the space by whoever turns up.

Keep it alive through change

New vessels, modifications, process changes, repurposing and even relocation of equipment create or alter spaces. Link the register to management of change so that a modification raises the question, because a space created incidentally will otherwise appear on the register only after someone has entered it.

Worked case

Case in point: the pit that was not on any list

A plant carried a confined space register covering fourteen spaces: tanks, silos, a blend vessel and two sumps. It was reviewed annually, spaces were signposted, permits were issued properly and training was current. The programme was well run.

During a floor repair a maintenance fitter removed a steel plate to access a cable route and dropped into a void beneath the plate, roughly a metre deep, to reach a junction box. The void ran beneath a section of the production floor and had accumulated a heavier-than-air vapour from a nearby transfer operation.

The void was not on the register, not signposted and not classified, because it had been created during a floor alteration years earlier and existed on no drawing. It met the definition of a confined space in every respect, and nobody had ever walked the site asking the question.

Frequently asked questions

How do we find all the confined spaces?+

By walking the site with the definition in mind, lifting covers and opening panels. Drawings show designed vessels and miss the spaces created incidentally: voids under floors, chambers behind equipment, ducts, pits from alterations. Those are consistently the ones that catch people, because they are unmarked, unclassified and unlisted.

When can a space be reclassified as non-permit?+

Only when every hazard within it has been eliminated, and the determination must be certified. Continuous forced ventilation eliminates nothing: switch it off and the atmosphere returns. A space kept safe by a running fan is controlled rather than eliminated and remains permit-required, which is among the most consequential classification errors in the standard.

What are alternate procedures?+

Entry under 1910.146(c)(5) where the only hazard in the space is atmospheric and continuous forced ventilation alone maintains a safe atmosphere for entry. It has defined conditions including monitoring and documentation, and it does not apply where any non-atmospheric hazard is present, such as engulfment or mechanical hazards.

What should each register entry contain?+

Location and identifier, classification with the reasoning, hazards present, previous contents, isolation points, access dimensions and orientation, whether retrieval equipment can physically be used, and rescue considerations. Without those an entry has to be planned at the space by whoever arrives, using whatever they can work out.

How often should the register be reviewed?+

On change rather than on a calendar. Modifications, new installations, process changes and repurposing all create or alter spaces, and none of them announce themselves as confined space events. Linking the register to management of change is what keeps it accurate; an annual review finds what changed only if someone remembers it.
