Examination by a competent person for visible defects, before each work shift and after any occurrence that could affect the scaffold's structural integrity, with authority to take prompt corrective action including removing the scaffold from service.
Who can perform it?
A competent person: someone capable of identifying existing and predictable hazards and authorised to take prompt corrective measures. That is a different role from the qualified person who trains employees under 1926.454, and the two are frequently conflated.

Check the base and the ties as carefully as the platform, and make the tag mean something.
Base plates and mudsills, level and undisturbed footings, and whether anything has been excavated, loaded or flooded near the base since the last inspection. Scaffold failures usually originate below the platform, and a base that has settled or been undermined is invisible from where people are working.
Ties removed to allow access for other trades and not replaced are a recurring finding, and the person who removed one is rarely the person who inspects. Check the tie pattern against what the structure requires, particularly where the height to base ratio exceeds four to one and the scaffold depends on them.
Fully planked with gaps no greater than one inch, minimum eighteen inches wide, no makeshift additions. Boards moved to reach something and not replaced, and buckets or step-ups placed on the platform to gain height, are both common and both invisible from ground level.
The tag at the access point is how the inspection reaches the people who use the scaffold. It should show who inspected it, when, and whether the structure is complete, incomplete or out of service. Where scaffolds carry no tag, or a permanently green one, the inspection informs the inspector and nobody else.

Worked case
Case in point: the tie that went missing
A scaffold on a building refurbishment was inspected each morning and tagged. On the third week a mechanical contractor needed to bring a duct section through the facade and removed two ties at second lift to clear the opening. The duct went in, the opening was closed, and the ties were not replaced.
The morning inspection the next day recorded the scaffold as satisfactory. The inspector checked the platform, the guardrails, the access and the base, and did not count ties against the pattern the structure required, because the scaffold had been tied correctly when it was erected and nothing visible had changed.
The height to base ratio at that location was well beyond four to one, and the structure was relying on ties it no longer had.

Frequently asked questions
How often must a scaffold be inspected?+
Before each work shift, and again after any occurrence that could affect its structural integrity. The second trigger is separate and immediate: a storm, a vehicle impact, material dropped against the structure, a tie removed, or ground disturbance at the base. Work should not resume on the strength of yesterday's inspection.
Why is the fall protection height ten feet rather than six?+
Because the scaffold standard sets its own threshold. Most construction fall protection under 1926.501 applies at six feet, while 1926.451(g)(1) requires protection for employees more than ten feet above a lower level on scaffolds. The difference is real, it catches people out in both directions, and suspended scaffolds carry additional requirements regardless.
What is most often wrong on a scaffold?+
Incomplete planking and gaps, missing or wrong-height guardrails, cross braces used for access, makeshift platforms added to gain height, and ties removed by other trades. The last is the most dangerous and the least visible, because the structure looks unchanged and nobody records the removal.
