Work Order Closeout Template

Closeout is where a maintenance job becomes a production hazard. Guards removed and not refitted, interlocks defeated for testing and left defeated, a tool inside a machine, a lock removed before the work was finished. The person injured is usually not the person who did the work, and the closeout is the only step designed to catch any of it.

The final step of a maintenance job, verifying that the work was completed and tested, that everything removed or defeated has been restored, that the area and the equipment are clean and clear, and that the asset is formally handed back to operations.

How is it different from completing the work?

Completing the work establishes that the fault is fixed. Closeout establishes the state the equipment and the area are being left in, which is a different question and the one that determines whether the next person to use the machine is safe.

Restore what was removed, prove what was defeated, account for what was brought in, and hand back to a person.

Verify restoration item by item

What was removed, what was defeated, what was disconnected, and confirmation that each has been put back and checked. A general statement that the machine has been reassembled is not verification. The technician knows what they took off; the record is where that knowledge becomes available to everyone else.

Prove safety functions, do not observe them

An interlock refitted and untested looks the same as one that works. Open the guard and confirm the machine stops or will not start. Where a safety function was defeated for testing, proving it afterwards is the step that closes the loop, and it takes a few seconds.

Account for tools and materials

Particularly in food and pharmaceutical areas, but universally worth doing. Tools brought in and out, fasteners removed and replaced, swarf and debris generated, cloths and consumables used. A tool left inside a machine is a foreign body incident, a damage event and an injury risk depending on where it ends up.

Hand back to a named person

Someone from operations accepts the asset, knowing what was done and any residual limitation. This creates a defined point at which responsibility transfers, rather than a period where the technician has left, production has restarted, and nobody has confirmed the machine is fit to run.

Worked case

Case in point: two of six bolts

A guard panel was removed to access a drive during a planned service. The work was completed, tested and the machine returned to production the same evening. The work order recorded the task, the parts and the time.

The panel had been refitted with two of its six fasteners, which held it in place and looked correct. Three weeks later the panel was disturbed during cleaning and moved enough to expose the drive while the machine was running.

Nobody had done anything unreasonable. The technician refitted the panel late in a job that had overrun, the closeout recorded that the work was complete, and no step in the process asked whether what had been removed was properly restored.

Frequently asked questions

Why is closeout separate from completing the work?+

Because they answer different questions. Completion establishes that the fault is fixed. Closeout establishes the state the equipment and area are being left in: what was removed and restored, what was defeated and proven, what was brought in and taken out. The second determines whether the next person to use the machine is safe.

Why do guard reinstatement failures recur?+

Because the person who removed the guard is the only person who knows it was removed, and the failure is usually partial rather than total: a panel refitted with two of six fasteners looks correct. The person affected is later, different, and has no reason to check. Listing removals at the start and checking them off at the end is what catches it.

Should interlocks be tested at closeout?+

Yes, wherever one was defeated, disconnected or disturbed. A refitted interlock and a bypassed interlock are visually identical, and the only way to distinguish them is to open the guard and confirm the machine stops or cannot start. It takes seconds and it is the difference between a restored safety function and an assumed one.

What does handback achieve?+

A defined point at which responsibility transfers. Without it there is a period where the technician has left, production has restarted and nobody has confirmed the machine is fit to run or communicated any limitation from a partial repair. Naming the person who accepts the asset closes that gap.

Because that is when it is known and it is never recovered later. The reliability history of an asset is built from these fields, and they are skipped most often on jobs that overran or finished under pressure, which are disproportionately the jobs on assets that fail repeatedly.
