What is a return to work programme review?
It is a periodic, programme-level review that aggregates every case handled in the period, rather than reviewing any single worker's progress. It looks at volume, timeliness, outcomes, where cases stall and whether the programme is treated fairly, and concludes with whether the programme itself is effective.
How is this different from a case-level progress review?
A case-level review, such as the Return to Work Progress Review, checks one worker's plan against their recovery. This review never opens a single case file; it works from the totals and patterns across all cases to find structural problems no individual case would surface alone.
Who carries this review out?
Occupational health, working with HR and management, because the questions span clinical timeliness, managerial cooperation with duties, and the fairness of how the programme was experienced. No one of those three functions sees all of it alone.
Scope
When is a return to work programme review required?
This review sits above the individual case, filled in from the accumulated record of every case in the period rather than any one worker's file, and it feeds management review rather than a single case decision.
Use this template when
- The stated review interval for the whole return to work programme has arrived
- A trigger event, a spike in reopened cases or a cluster of stalled cases, calls for the programme to be examined out of cycle
- The workspace is being set up and the programme needs its first baseline review recorded
- Management review needs this record to exist as its input on return to work performance

Return to Work Programme Review requirements by country
Most jurisdictions regulate the return to work duty case by case, through reasonable accommodation or adjustment obligations; a programme-wide review like this one is driven far more consistently by the management system standard than by statute.

Reasonable accommodation obligations attach case by case; there is no statutory requirement to review the return to work programme as a whole.
A pattern of stalled cases concentrated in one department can expose an employer to accommodation claims even where no single case was mishandled, which is what a programme-level review is positioned to catch first.
Equality Act 2010 reasonable adjustments; HSE guidance on managing sickness absence
Reasonable adjustment duties are assessed case by case; HSE guidance encourages oversight of absence and return trends but does not mandate a programme review by name.
A tribunal reviewing a failed adjustment will look at whether the employer knew of a pattern, such as one area consistently refusing suitable duties, making this review evidence of what was knowable.
ISO 45001:2018 clause 9.3, management review
Management review must receive the results of OH&S performance evaluation, including the effectiveness of programmes such as return to work.
Auditors will expect this review's conclusion, and any stated gap, to actually reach management review rather than stopping at occupational health.
How to complete it
How to complete a return to work programme review, step by step
The counts here are mechanical. The judgement calls are in how they are read, and whether a comfortable overall conclusion survives a look at where the numbers concentrate.
Cases Opened, Cases Closed and Cases Still Open have to add up against the carry-forward from the previous period. A review built on a count that does not reconcile has started from an unreliable base.
Six stalled cases spread across six areas is a different problem from six stalled cases in one area. Cases Held More Than Twice and Stalling Concentrated In One Area have to be read together.
Confidentiality Maintained Throughout is scored zero for even one breach, deliberately, because trust in the programme depends on it absolutely. A programme that returns people quickly but leaked one worker's diagnosis has not had a good year here.
Programme Effective can be ticked 'Yes' in the same record that names a Main Gap. The reviewer has to decide whether the gap is minor enough to coexist with 'effective' or should be answered 'Partly' instead.

Findings against this review rarely concern whether the counts were entered. They concern whether the counts were read for the pattern they contain, and whether an uncomfortable pattern reached management review at all.

Case in point
Case in point: the programme that looked effective on paper
An annual review recorded a reasonable average days-to-return figure, most areas cooperating with suitable duties, and a low reopened-case count. Programme Effective was ticked 'Yes', Stalling Concentrated In One Area was ticked 'No', and the record moved to management review without further comment.
A closer look the following year, prompted by an unrelated exit interview, found one warehouse area had not taken a single restricted worker in three years. Its cases never showed up as stalled, because instead of waiting in suitable duties for a placement, workers from that area were consistently redeployed or exited the business, leaving the process rather than stalling inside it. The stalling measure had no way to see a department that never entered the pipeline it was designed to watch.
